Providing one of the following documents with the applicant’s name and address is acceptable as proof:
- Maryland Driver’s License
- Maryland vehicle registration card or title
- Voter registration card
- Utility or credit card or other bill
- Checking or savings or retirement account statement
- Lease agreement or contract
- Mortgage account or proof of home ownership
- Installment loan contract from a bank or other lender

Are you able to provide proof of US citizenship or legal permanent resident status?
Providing one of the following documents with the applicant’s name is acceptable as proof:
- U.S. Passport
- U.S. Birth Certificate
- Certificate of Naturalization
- Certificate of Citizenship
- Green Card or Permanent Resident Card
